For students at UQ, QUT, Griffith, UniSC, and CQU
If you are a nursing or midwifery etc student preparing for clinical placement at a university in Queensland (such as UQ, QUT, Griffith, UniSC, or CQU), you are required to complete vaccination and immunity verification as part of your placement eligibility. This process typically involves providing a documented history of immunisations and, in many cases, supporting blood test (serology) results to confirm immunity to specific diseases.
Please note that this process usually requires two consultations:
- An initial appointment to review records, order any necessary blood tests, and administer vaccines if needed.
- A follow-up consultation to review test results and administer vaccines if needed and complete the official university verification form.
Gather Your Immunisation Records
Bring the following documents to your GP appointment:
- AIR (Australian Immunisation Register) history or overseas equivalent
- Vaccination certificates (e.g. Hepatitis B, MMR, dTpa, Varicella, Influenza, COVID-19)
- Any existing serology results (blood tests for immunity)
If your records are in a foreign language, provide an English translation, preferably by a NAATI-certified translator.
Book a GP Appointment
Let the clinic know the appointment is for nursing or midwifery etc placement vaccine clearance. Bring:
- Your university’s official vaccination or health clearance form
- Your immunisation history and serology results
- Any translated documents
During the consultation, the GP will:
- Review your vaccine history
- Order serology blood tests (e.g., Hep B antibody titres) if required
- Administer any missing vaccines
- Complete, stamp, and sign your university’s immunisation form
Note: If you need additional vaccinations, they must be paid for privately, as they are usually not covered by Medicare unless part of the national immunisation program.

Submit Your Documents
- Upload your completed form, serology results, and any other documents to your university’s portal (e.g. SONIA, HiQ)
- Keep both digital and printed copies
- Submit by the university’s deadline (often 6–8 weeks before placement)
Final Reminders
- Ask your GP to stamp, sign, and date all documentation
- Begin early to allow time for:
- Vaccine series (e.g., Hep B takes months)
- Serology processing
- Any necessary repeat vaccinations
- Ensure all foreign records are properly translated
- Confirm which documents your university requires
